The relation of frequency of episodes of painless ischemia on 24-hour
ECG to the time between registration of ST-seqment depression and emer
gence of angina during bicycle exercise test was studied in 45 men wit
h stable class II - III effort angina. In most of the patients with ea
rly (relative to registration of 1 mm ST-segment depression) appearanc
e of angina during exercise test number of episodes of painless insche
mia did not exceed 25% of total number of episodes of ST-segment depre
ssion. Meanwhile in about half af;he patients in whom time gap between
registration of 1 mm ST-segment depression and onset on angina was eq
ual to or exceeded 60 sec more than 75% of ischemic episodes were pain
less. Thus time interval between the onset of attack of angina and app
earance of 1 mm ST-segment depression during bicycle exercise test is
a parameter which allows to identify a group of patients with predomin
antly painless ischemic heart disease and a group of patients with low
threshold of ischemic pain perception.
