A new case of bilateral diabetic papillopathy occurring in a 43-year-o
ld woman with poor metabolic control of type 1 diabetes is presented,
The clinical picture included a bilateral macular involvement related
to choroidal folds with macular distortion and a peripapillary serous
neuroretinal detachment, appearing before the onset of disc swelling.
The clinical data concerning diabetic papillopathy are reviewed, and t
he relationships between this disease and metabolic disturbances are d
iscussed The peripapillary serous neuroretinal detachment is supposedl
y consecutive to an acute and reversible breakdown of the blood retina
barrier, whose onset is possibly consecutive to glycemic variations,
hormonal changes, or immunological factors.
