A new procedure to close extensive defects on the perineal area after
vulvectomy is described. For years, the three first authors of this pa
per performed a number of surgical reconstructions after vulvectomy em
ploying a personal technique that was described in 1987. But this tech
nique is not useful when the excision has to be performed far beyond t
he anal orifice, involving a great part of the thighs. In this paper w
e describe our procedure of double rotation-transposition flaps design
ed to repair a great defect following extensive excision. As a conclus
ion, we wish to stress out that the rotation-transposition flaps shoul
d always be the technique of choice when repairing defects around the
natural orifices. Grafts will only be useful to cover the donor area o
