We introduce a combined vaginal and hysteroscopic approach to the trea
tment of a special class of septate uterus: complete uterine septum wi
th cervical duplication and longitudinal vaginal septum, In a 22 year-
old women with a 4-year history of infertility, a transcervical lysis
of the complete septum with an operating resectoscope and complete inc
ision of the vaginal septum was performed, A pediatric No, 8 Foley cat
heter was inserted in the right cervix, The left cavity was distended
with 1.5% glycine via the resectoscope. The septum electrosurgically i
ncised at a point above the internal cervical os until the Foley cathe
ter was visualized, The uterine septum was incised in a superior direc
tion until the tubal ostium could be visualized, After the hysteroscop
ic procedure, the vaginal longitudinal septum was incised completely,
The result was unification of two uterine cavities and the vagina, con
ception within 5 months, and ongoing pregnancy (30 weeks), Our patient
had a rare uterine deformity, This procedure enables transcervical me
troplasty for a complete septum in patients who have had repeated repr
oductive wastage and primary infertility.
