For a subset of impotent patients with penile fibrosis, the implantati
on of a penile prosthesis is their only therapy for restoration of ere
ctile function. A careful preoperative evaluation is imperative and sh
ould include a detailed history, physical examination, selected diagno
stic studies, selection of a prosthesis, and extensive patient and par
tner counseling. Techniques for managing fibrosis are outlined, and in
many cases, additional reconstructive procedures may be required. The
se surgical techniques permit highly motivated patients not to termina
te their sex lives at an unacceptably early age.
