Between 1971 and 1994, 27 patients with scar tissue and fistula carcin
omas were operated on at the Berufsgenossenschaftliches Unfallkrankenh
aus Hamburg. Of the seven patients who deceased, 4 died because of the
ir carcinoma. The mean latency between injury and manifestation of the
carcinoma was 27.2 years. The mean follow-up time of the 20 survivors
was 7.2 years, all of them are without recurrent disease. Nine local
excision were performed, one below-knee amputation was subsequently ne
cessary and 15 primary amputations were carried out. The three decubit
al carcinomas were so extensive that a complete resection could not be
achieved. Lymphadenectomy is recommended if positive lymphnodes are p
resent. Prophylactic lymphadenectomy appears to be helpful in the case
of squamous-cell carcinoma subsequent to burns. Postoperative radiati
on is not indicated.
