In 5 patients with isolated left main coronary artery disease the foll
owing direct reconstructive procedures were performed: autovenous graf
ting, instrumental dilatation, and autovenous patch implantation. One
patients died of perioperative myocardial Infarction. In four other pa
tients surgery resulted in improvement of results of exercise tests an
d disappearance of angina; These patients were followed up from 2 mont
hs to 3 years and none of them developed myocardial infarction during
this period. Good patency of reconstructed segments of left coronary a
rtery trunk was found at control angiography in all 4 cases.
