A young patient with testicular germ cell tumor presenting with inferi
or vena cava thrombus extending into the right heart with free-floatin
g thrombus in the right ventricle and a simultaneous epidural spinal c
ord compression is presented. Due to the perceived high risk of emboli
zation and the urgent need to begin systemic chemotherapy, he was mana
ged with tumor thrombectomy utilizing cardiopulmonary bypass and hypot
hermic circulatory arrest followed shortly thereafter by systemic chem
otherapy. There were no perioperative complications, and he is alive a
nd without recurrence 24 months following four cycles of systemic chem
otherapy.
