Abstract
BACKGROUND Giant or large aneurysms prevent direct clipping without co mpromise of the parent vessels, and any countermeasures should be atte mpted. METHODS We describe an A3-A3 side-to-side anastomosis as a meth od of revascularization of the pericallosal artery in surgery of an an eurysm of the anterior communicating artery (ACoA) in four patients. R ESULTS In two patients with the pericallosal artery narrowed or occlud ed by the clipping or trapping procedure, and in two other patients wi th giant aneurysms clipped with prolonged duration of temporary occlus ion of the parent vessels, no serious neurologic changes were observed after surgery. CONCLUSIONS We believe that an A3-A3 side-to-side anas tomosis is effective in preventing ischemic complications in the terri tory of the pericallosal artery.
