R. Fricker et al., ACUTE MEDIAN NERVE COMPRESSION AT THE DISTAL FOREARM CAUSED BY A THROMBOSED ANEURYSM OF AN EPINEURAL VESSEL - CASE-REPORT, Neurosurgery, 38(1), 1996, pp. 194-196
THE CASE OF a patient with a 2-day history of symptoms suggesting acut
e carpal tunnel syndrome is presented. However, an urgent electroneuro
graphic examination revealed median nerve compression at the forearm a
nd magnetic resonance imaging confirmed compression by a mass proximal
to the carpal tunnel. Surgical exploration showed a recently thrombos
ed aneurysm of an epineural vessel. Histological and, later, general a
nd angiological investigations could not reveal the underlying cause o
f this aneurysm. Preoperative electrodiagnostic examination is recomme
nded in acute peripheral nerve compression to prevent decompression at
an incorrect site. If atypical nerve compression is suspected, magnet
ic resonance imaging may be indicated to detect localized nerve compre
ssion and its underlying cause.
