Porokeratotic eccrine ostial and dermal duct nevus (PEODDN) is a rare
disorder characterized by pits and keratotic papules appearing usually
at birth. Histologically, PEODDN is characterized by parakeratotic co
rnoid lamellae in the eccrine ducts. We present here a case of the sys
tematized form of PEODDN in a 23-year-old man with human immunodeficie
ncy virus (HIV) infection. The patient had a 2-month history of kerato
tic and verrucous papules and nodules, distributed over all extremitie
s; a group of punctate pits were present on the soles. PEODDN adds to
pulosquamous dermatoses that develop in HIV-positive patients in assoc
iation with a marked immunosuppression. Immunosuppression could act as
trigger factor and precipitate the onset or the exacerbation of these
porokeratotic hamartomatous lesions thus explaining the unusual clini
cal features and the extensive involvement observed in our patient.
