The Simulation Index-Revised (SI-R) was previously developed to differ
entiate impaired motivation from traumatic brain injury sequelae in pa
tients referred for neuropsychological assessment. Rather than using q
uantitative comparisons of performance, the SI-R evaluates qualitative
errors on the WAIS-R and WMS. The present study examined the specific
ity of the SI-R in a clinical sample of 338 patients of mixed diagnose
s. The overall level of specificity was 62% in this sample. patient gr
oups with diagnoses of schizophrenia, vascular dementia, and alcohol d
ependence had lower specificity than did the mean of the entire sample
. The need for additional studies and SI-R item analyses are discussed
