Acquired inhibitors of factor VIII are rarely observed as a life threa
tening complication of advanced neoplastic diseases. Profuse bleeding
and isolated PTT-prolongation raise the suspicion for this coagulopath
y. In this Case Report the authors describe the appearance of a plasma
tic circulating antibody against factor VIII in advanced cervical carc
inoma. Local therapeutic measures to arrest the bleeding or high-dose
factor substitution did not influence the clinical outcome of the pati
ent substantially. In our experience only the combination of immunsupp
ressive treatment (cyclophophamide/prednisolone) with FEIBA and AT III
has proved to be effective for cessation of the clinical symptoms and
sufficient suppression of serum levels of the inhibitor.
