A case of syndrome of inappropriate antidiuretic hormone secretion (SI
ADH) in a patient with suboptimally cytoreduced stage III papillary se
rous surface carcinoma of the peritoneum is described. After the prima
ry surgery, the patient refused further therapy. Within a month bilate
ral pleural effusions and abdominal ascites compelled the patient to a
ccept treatment with carboplatin and cyclophosphamide. Ten days follow
ing the chemotherapy, she was admitted in a disoriented state with ser
um sodium of 117-mEq/L. During the evaluation, treatment, and subseque
nt followup, the diagnosis of SIADH was confirmed. Numerous disease pr
ocesses have been associated with the development of SIADH; however, t
here have been few reports in gynecologic malignancies. Possible etiol
ogy and clinical management of this patient are briefly discussed. (C)
