We describe the first reported case of an internal mammary artery orig
inating from the junction of the left subclavian artery and aorta. Not
ing this, along with other reported anomalies and various pathologic c
onditions, it appears warranted to perform routine preoperative intern
al mammary artery angiography before coronary artery bypass surgery. (
