Subscapular elastofibroma is one of the differential diagnoses of tumo
rs near the lower border of the scapula. The tumor is benign and radic
al surgical resection is the treatment of choice. However, this tumor
mimics a soft tissue sarcoma, both on clinical grounds and on preopera
tive MR imaging. Awareness of the benign nature of subscapular elastof
ibroma is important to avoid unnecessary rib cage excision and reconst
ruction.
