A 68 year-old man underwent surgical repair of a ventricular septal de
fect following an acute myocardial infarction. Recurrent interventricu
lar septal rupture with significant left-to-right shunting led to prog
ressive deterioration in cardiac function and intractable heart failur
e. The patient underwent orthotopic heart transplantation three months
after his initial operation, and he is clinically well sixteen months
after transplantation.
