The authors report 4 cases of symptomatic dural fistula of the carotid
cavernous sinus treated by transvenous coils. The objectives of dural
fistula treatment are now clearly established and several methods are
available. Abstention is the rule for asymptomatic forms, whereas sym
ptomatic forms with ophthalmological complications may respond to manu
al compression of the jugular vein and the carotid artery, arterial em
bolization, radiotherapy or venous embolization.Arterial embolization
results in complete cure in 72 % to 78 % of the cases, but it is limit
ed by the meningeal supply of the internal carotid artery, with the mi
crocatheter the transvenous route makes it possible to treat the menin
geal supply from the internal and external carotids without affecting
the arterial system, but this technique is restricted by the anatomy o
f each patient.
