Nine patients with Moyamoya disease were followed by the authors betwe
en 1982 and 1991. The patients were investigated with transcranial Dop
pler sonography (TCD), angiography and clinically. Two patients were t
reated conservatively. Bilateral extra-intracranial (EC-IC) anastomosi
s was performed in 4 patients; in 2 patients bypass surgery was perfor
med unilaterally. One patient with an aneurysm of the carotid artery p
resented a subarachnoidal haemorrhage. There was a good correlation be
tween TCD and angiographical findings in all patients. In four patient
s indication for bypass surgery was established by TCD. All patients w
ho underwent EC-IC bypass surgery improved clinically; the symptoms di
sappeared in three patients and improved markedly in two patients. Tra
nscranial Doppler sonography is proven to be a reliable atraumatic too
l in diagnosis, indication for treatment and postoperative follow-up i
n patients with moyamoya disease.
