A 35-year-old man developed rapid progressive psychosyndrome and coma
over 3 weeks. There were no other neurological deficits. Seroscopy and
cerebrospinal fluid examination revealed no signs of inflammation or
intoxikation. In the further course of the disease intracerebral haemo
rrhage and sepsis occured. The patient died after 5 weeks. The autopsy
revealed a subcortical necrotizing encephalopathy. With respect to a
hypertrophy of the left ventricle anamnestic unknown arterial hyperten
sion could be postulated. Therefore Binswanger's syndrome was diagnose
d. This syndrome should be respected in differential diagnostic of rap
id progressive psychosyndrome even in young patients.
