Direct and indirect measures of the reliablity of the transfusion proc
ess are described. These measures can be used to assess the improvemen
t of the transfusion process with a view to preventing hemolytic incid
ents. Quality assurance arrangements required by their use are made cl
ear. The stress is put upon four points: processes must be formalised
and standardised; quality audits must become a routine part of the tra
nsfusion process; the system of error reporting must be extended to in
clude all failures; anonymity must be insured to improve reporting.
