The role of risperidone in the management of treatment-resistant schiz
ophrenia remains unclear. We describe two patients with treatment-resi
stant schizophrenia characterized by unremitting delusions, thought di
sorder and self-neglect who had a sustained improvement in their sympt
oms soon after starting risperidone. Risperidone may be a suitable alt
ernative to clozapine in some patients unresponsive to conventional an
tipsychotics.
