Abstract
The use of the iliac crest flap based on the deep circumflex iliac art ery and vein is well established in orofacial reconstruction. In cases where the deep circumflex iliac artery and vein are of inadequate siz e, or when large composite flaps are required, vascular augmentation c an be achieved using the iliac branches of the iliolumbar artery and v ein as an additional pedicle. Routine inclusion of this additional ped icle in iliac crest naps creates extra safety with a low effort.
