Affective disturbances or paranoid syndromes are very well known in di
fferent somatic diseases. A depressive syndrome with psychotic feature
s as psychopathological symptoms of a neurolues is described rarely in
the literature. We report about a male depressed inpatient, 64 years
old, admitted because of delusional depression with hypochondrical ide
as and a periodical course. In the laboratory test lues was positive,
vitamin B12 showed a borderline deficiency, a subclinical hyperthyroid
ism was found. The inpatient received treatment with antidepressants a
nd antipsychotics, penicilline intravenous and a substitution of vitam
in B12 which led to a complete improvement of delusional symptomes. At
the end of treatment an organic psychosyndrome with mild depressive m
ood was seen. Course of therapy, treatment complications and clinical
picture are discussed as well as possible relations in the sense of co
morbidity and etiology between an organic infective disease of the CNS
and depressive episodes.
