A 10-year review (1984 to 1994) of patients undergoing surgical treatm
ent for hyperparathyroidism at St. Joseph's Health Centre in London, O
ntario, is presented. The role of preoperative localization is discuss
ed. We review our experience with the various imaging modalities and t
heir usefulness in the surgical decision-making process. Our experienc
es with the surgical management of both primary and secondary hyperpar
athyroidism are evaluated. Controversies in parathyroid surgery, unila
teral versus bilateral neck exploration for parathyroid adenomas, and
autotransplantation versus total parathyroidectomy for secondary hyper
parathyroidism are discussed and our approach is explained.
