A case secondary mania occurring in a 64-year-old women on two separat
e occasions following surgery under local lignocaine anaesthesia is re
ported. The episodes of mania were temporally related to the surgery a
nd fulfilled DSM-III-R criteria. The possibility of a reaction to lign
ocaine is discussed and it is concluded that the mania was secondary t
o the physical insult of the surgical intervention.
