Background: Myocardial infarction is a rare event in pregnancy that re
quires medical stabilization with possible surgical interventions. Cas
e: A 37-year-old woman developed an anterior wall myocardial infarctio
n secondary to severe coronary artery disease during the early third t
rimester of pregnancy. The patient was managed with multiple modalitie
s to stabilize her condition and prolong the gestation. The salient po
ints in the management included: defibrillation and pharmacologic agen
ts, intra-aortic balloon pump, coronary artery bypass graft surgery us
ing a cardiopulmonary bypass pump while maintaining continuous fetal h
eart rate monitoring, and delivery of the neonate at term. Conclusion:
Aggressive management of this patient allowed continuation of pregnan
cy with good outcome for mother and neonate.
