Renal cholesterol embolism is a rare disease with a high mortality occ
urring as a complication of severe generalized arteriosclerotic diseas
e. We report the case of a 65 year old man who developed systemic chol
esterol embolic disease and acute renal failure after coronary angiogr
aphy. He presented with hypertension, livedo reticularis, finger infar
cts, progressive renal failure requiring dialysis, eosinophilia, eosin
ophiluria and retinal emboli; the skin biopsy revealed cholesterol cle
fts in the dermal vasculature surrounded by inflammatory infiltrate. E
ight months later the patient remains on hemodialysis and he has surge
ry of coronary vessels and abdominal aortic aneurysm, with no complica
tions.
