A case is presented of a 38-year-old male with dextrocardia in whom ra
diofrequency current ablation of an incessant atrial tachycardia origi
nating within the infero-lateral pulmonary vein was achieved. Activati
on mapping with defection of the earliest atrial activation was used f
or identification of the arrhythmogenic focus. in addition to fluorosc
opy, transesophageal echocardiography was used for catheter guidance d
uring the transseptal puncture. The present experience suggests that l
ocation of an arrhythmogenic focus within the pulmonary venous system
should be considered whenever early atrial activation during ectopic a
trial tachycardia is recorded at the junction between the left atrium
and the pulmonary veins.
