The differential diagnosis of tachycardias with a wide QRS complex inc
ludes ventricular tachycardia, supraventricular tachycardia with bundl
e branch block aberration and supraventricular tachycardia with ventri
cular preexcitation in WPW syndrome. In all hemodynamically tolerated
tachycardias with a wide QRS complex, a 12-lead standard-ECG should be
obtained before terminating the tachycardia. The careful analysis of
the 12-lead standard-EGG using the ECG criteria described in the prese
nt review leads to the correct diagnosis of 80-90% of all tachycardias
with a wide QRS complex.
