The high incidence (70%) of anterior hypospadias, mostly without penil
e curvature, in our pediatric hypospadias patients led us to study the
meatal location in normal adults. The location of the external meatus
was analyzed in 500 adults (mean age: 57) with classification of the
meatal position in relation to the tip of the glans and corona. Qualit
y of erections and sexual intercourse, presence or absence of penile c
urvature, urinary stream and ability to void in a standing position we
re assessed in an interview. Among the investigated 500 adults only 55
% (275) were normal, with the meatus in the distal 1/3 the glans, whil
e 65 (13%) asymptomatic anterior hypospadias (glanular: 49, coronal: 1
5, subcoronal: 1) were observed. 32% (160) showed a position of the ur
ethral meatus in the midthird of the glans. These observations appear
to question the need for meatal advancement in anterior hypospadias wi
thout associated penile curvature.
