The additional use of the palmaris longus tendon is presented with the
composite radial forearm-palmaris longus free flap, as useful for imp
roving the final cosmetic appearance. This flap has the additional adv
antage of avoiding gravitational descent, which is a common problem en
countered with many free naps in head and neck reconstructions. A clin
ical case is also presented as an example of this approach.
