PURPOSE: To describe two patients with uveitis who developed increased
intraocular pressure that was unresponsive to maximum medical therapy
eight and 13 months after periocular injection of triamcinolone aceto
nide. METHODS: Excised periocular tissue was analyzed for corticostero
id activity by gas chromatography and mass spectrometry. RESULTS: Exci
sion of the periocular tissue, which contained visible triamcinolone a
cetonide, result ed in a normal intraocular pressure within 14 days. i
n both patients. Analysis of the excised tissue disclosed residual cor
ticosteroid in one of the two patients. CONCLUSION: Removal of periocu
lar tissue containing injected corticosteroids may facilitate the mana
gement of patients developing increased intraocular pressure unrespons
ive to maximum medical therapy.
