The patient described has a history of recurrent gestational trophobla
stic disease following spontaneous conception, She subsequently underw
ent two cycles of in-vitro fertilization (IVF) for management of infer
tility related to tubal obstruction, IVF of the oocytes retrieved show
ed a significantly high incidence of abnormal fertilization resulting
in the development of triploid embryos. This report explores the possi
ble association of an oocyte defect predisposing to abnormal fertiliza
tion, resulting in a high incidence of triploid embryos, Since the dev
elopment of partial hydatidiform moles is related to the origin of tri
ploidy, this phenomenon is suggested to explain the occurrence of recu
rrent trophoblastic disease in this patient. We propose the use of int
racytoplasmic sperm injection (ICSI) as a therapeutic option to minimi
ze the incidence of triploidy in future IVF cycles; donor oocyte IVF w
ould be another alternative.
