We describe a patient with convergence-retraction nystagmus caused by
a hemorrhage in the right thalamus and dorsal midbrain. This was docum
ented by magnetic resonance imaging. Convergence-retraction nystagmus
has a high localizing value for a (most frequently tumorous or vascula
r) lesion near of within the dorsal midbrain. The convergent eye movem
ents have the characteristics of saccades; the retraction nystagmus is
caused by simultaneous contraction of all extraocular muscles.
