Mycotic aortic aneurysm is a rare yet life-threatening complication af
ter orthotopic heart transplantation. This article reviews three cases
of mycotic aortic aneurysm in heart transplant recipients developing
in the first year after heart transplantation. Excision of the aneurys
m and in situ reconstruction of the ascending aorta were performed wit
h a patch of glutaraldehyde-fixed pericardium or cryopreserved aortic
allograft material as a patch or conduit replacement. These cases show
that early diagnosis by computed tomographic scanning, surgical treat
ment, high-dose parenteral antibiotics, and close follow-up are essent
ial for successful treatment.
