This study presents measures of unilateral deficits on the Rey Figure
copy that may be easily incorporated into the scoring system described
by Lezak (1983, 1995), extending previous research that reported simi
lar indices using the Denman (1984) scoring system. One hundred and ei
ght subjects participated. Indices of unilateral inattention on the Re
y Figure were compared to criterion measures on the Letter Cancellatio
n Test. Left-side omissions on the Rey Figure were greater among right
-CVA neglecting subjects (n = 36) than among non-neglecting right-CVAs
(n = 32), left-CVAs (n = 20), and controls (n = 20). In addition, rig
ht-CVA subjects with neglect omitted significantly more items and evid
enced much poorer accuracy of reproduction on the left versus the righ
t side of the figure, whereas no unilateral differences were noted amo
ng the other groups. When using the Lezak-Osterrieth system, an asymme
tric error profile containing two or more left-sided omissions indicat
es a strong likelihood of neglect (97% correct classification). Left-s
ide omissions were significantly related to inpatient falls.
