A case is described of a clinically occult carotid artery lesion secon
dary to penetrating trauma. Angiography displayed a small intimal defe
ct, whereas surgical exploration revealed significant intimal damage r
equiring excision and interposition grafting. This case supports the b
enefit of continued definitive operative management for carotid artery
injuries.
