Ceftriaxone (Rocephin(R)) is often used in treatment of severe bacteri
al infections. The appearance of precipitations in the gall bladder is
a well-known adverse effect under Ceftriaxone therapy. To our knowled
ge only two cases of Ceftriaxone-assaciated nephrolithiasis have been
published before. We report two further cases of patients with renal s
ide effects under treatment with Ceftriaxone: A 6-year-old boy develop
ed nephrolithiasis with unilateral obstruction of the urinary tract, a
nd a 9-year-old girl an acute postrenal insufficiency with anuria. In
both cases it was documented by infrared spectroscopy and mass spectro
metry that urine stone-formations contained Ceftriaxone-compounds. Suf
ficient fluid supply is necessary to avoid renal side effects. In all
patients with hematuria or abdominal pain the urinary tract shoud be e
xplored by ultrasonography.
