Abstract
A patient presenting primary pulmonary hypertension and absent right s uperior vena cava underwent blade/balloon atrial septostomy as palliat ive therapy. Due to the anomaly of the venous drainage system, only tr ansesophageal echocardiography allowed the performance of the maneuvre .
