Abstract
We thank Georges Bresson, Ricardo Caballero, Ben Craig, Wolter Hassink , Steve Nickell, Patrick Sevestre, Peter Wilcoxen, and participants at seminars at several universities and conferences in North America and Europe, and a number of referees for their comments.
