A woman at 33 weeks' gestation was admitted because of the sudden onse
t of signs and symptoms suggestive of placental abruption. The fetal c
ardiotocogram was suspicious of fetal distress. Laparotomy revealed a
massive hemoperitoneum with splenic rupture, which was managed conserv
atively. Both mother and infant had a favorable outcome. We suggest th
at splenic conservation is a safe management option in selected cases.
