Abstract
The quality assurance programme in the North-Rhine area of Germany doc uments about 11 000 laparoscopic gall bladders per year and per ten mi llion inhabitants. In 1994 51 lesions of the common bile duct were doc umented. A structured anonymous questionnaire was sent to all the hosp itals that reported a common bile duct injury. The results of this que stionnaire were analyzed according to a failure analysis strategy. The aim was to develop a strategy to avoid common bile duct injuries in t he future.
