Abstract
Nurses face a significant challenge in meeting the diverse needs of cl ients who are moving more and more quickly from one care setting to an other. To address this challenge, organizations need to develop a well -established referral system to assist in the process of linking clien ts to community resources, and practitioners need to have an understan ding of the basic principles and clinical and theoretical aspects of t he referral process.
