The epididymal leiomyoma is a benign neoplasm that usually appears as
a slow growing, painless scrotal mass.(1-4) Although the leiomyoma is
the second most common neoplasm of the epididymis, detection frequentl
y leads to radical orchiectomy on the mistaken presumption of malignan
cy To date, sonographic characterization of these masses has been limi
ted.(5-7) Because the scrotal leiomyoma shares histologic features wit
h the more familiar uterine leiomyoma, it is perhaps not surprising th
at these two tumors should also share common sonographic features. We
describe the ultrasonographic characteristics of an epididymal leiomyo
ma that exhibited multiple recurrent sharp shadows that did not origin
ate from calcifications. To our knowledge, this pattern, common in lei
omyomas originating from the uterus,(8) has not been recognized previo
usly in leiomyomas in the scrotum.
