A 22-year-old man developed severe haematemsis 21 years after colon in
terposition for long-gap oesophageal atresia. A fistula, from an anast
omotic ulcer to the descending thoracic aorta. was discovered and trea
ted successfully by surgical resection. This previously unreported com
plication highlights the need for the prevention of peptic complicatio
ns following oesophageal replacement in children.
