We describe of a 55-year-old man with end stage renal disease secondar
y to renal ectopia and fusion (horseshoe kidney) and obstructive uropa
thy. After a year on maintenance hemodialysis, he developed acute hemo
lytic episodes, thrombocytopenia and positive antinuclear antibodies.
After the differential diagnosis had been explored, we concluded that
he had incomplete systemic lupus erythematosus. We emphasize the low f
requency of systemic lupus erythematosus in the chronic dialysis popul
ation, and the difficulty of making this diagnosis in there circunstan
ces.
