Abstract
Geriatric assessment has become an established part of medical practic e, a trend driven by the growing population of older patients, positiv e patient outcomes, and increased interest in controlling healthcare c osts, Geriatric assessments, is a diagnostic process that can be perfo rmed in a variety of clinical settings, including the primary care off ice. The interdisciplinary assessment team usually includes at least t hree members: a physician, a nurse, and a social worker. Patients who appear to derive the greatest benefit are over age 75, have mild to mo derate disabilities, may be at risk of nursing home placement, and may have a poor social network. For optimal effectiveness, assessment mus t be coupled with a comprehensive therapeutic plan and long-term follo w-up.
