A 27-year-old surgical nurse developed generalized urticaria and dyspn
ea. after wearing latex gloves for a short period of time. However, th
e tentative diagnosis of latex allergy could not be proven by latex pr
ick testing. Taking the patient's history once again revealed that she
had taken a combined preparation of propyphenazone 125 mg, dihydroerg
otaminmesilat 0,5 mg and coffein 40 mg (Optalidon special N) seven hou
rs before her symptoms began. Since propyphenazone and the other activ
e and inactive ingredients of Optalidon special N were negative in pri
ck testing we could identify propyphenazone as the causative agent onl
y by oral provocation. Other analgesics such as aspirin and paracetamo
l were negative in oral provocation tests.
