An 84-year-old man with a nuclear cataract had phacoemulsification, co
ntinuous curvilinear capsulorhexis, and implantation of a silicone int
raocular lens. Two months later, the patient had vision loss and sever
e anterior capsule fibrosis, leading to complete posterior chamber len
s encapsulation. After a capsular opening was created with a neodymium
:YAG laser, visual acuity returned to the preoperative level of 20/30.
